This 3 part carbonless specimen collection form is a complete chain-of-custody documentation solution designed for workplace drog-of-abuse (MRO) and breath-alcohol testing, fully match international specimen testing custody-of-evidence standards. It is uniquely serialized with barcode security labels and tamper-evident security seals. It creates a full, traceable audit trail from sample collection, donor declaration, collector certification, through to laboratory receipt, complying with international specimen testing custody-of-evidence standards.

1.Triplicate NCR carbonless copy set
Three page layout: Collector copy, Donor copy and Laboratory copy. No carbon paper is required; information written on the top sheet automatically transfers to the underlying copies for simultaneous record-keeping.
2. Full-process step-by-step workflow
Pre-printed sections guide users through 5 defined stages: Donor information declaration, medication disclosure, collector validation, lab receiving confirmation and MRO result review, covering every link of specimen handling.
3. Unique specimen ID barcode system
Matching, pre-printed barcode stickers with identical serial numbers are supplied. Labels can be peeled off and affixed onto sample containers to guarantee one-to-one matching between paperwork and physical samples and eliminate mix-ups. After removing labels, the main form stays fully intact for lab filing.
4. Tamper-evident security seals
Custom security stickers marked PLACE (A) OVER CAP and PLACE (B) OVER CAP are used to seal specimen bottle lids. Once applied and removed, they cannot be re-sealed without visible evidence of tampering. Custom logo holographic security seals are also available.
5. Multi-sample-type compatibility
Suitable for Urine, Saliva, Hair and other biological specimen collections, widely used for marine, transport, corporate workplace, pre-employment and random alcohol screening programmes.
